Subscription Form Shows up strangely in NS 4.7

On this page, http://www.yqcomputer.com/ , there's a form for subscribing to a newsletter on the left hand side of the screen. It looks fine in NS 7 and IE 6, but when viewed on NS 4.7, the labels for the fields disappear and the fields appear much longer than in current browsers. This forces the subscribe button off the screen when resolution is set at 800x600. When I run the browser check in DreamWeaver, it doesn't show any related errors for 4.7. Anyone have any ideas?
