Assuming your isp does not receive mail for you, that the mail comes
directly to your server, then all you have to do is make sure there is an mx
record on the internet that resolves to your public ip address and any
traffic that hits that public ip address destined for port 25 is directed to
your exchange server. If you were using the isp as a smarthost rather than
doing dns lookups for each external domain you will need to update that
entry on the IMS to your new ISP's smarthost.
